New Patients
If you are a new patient please fill out paperwork from the drop down menu. This is submitted electronically. You can also come in 15 minutes early to fill this out at your appointment. Please do not fill out paperwork if you have not scheduled an appointment first.
Insurance
Oceanpoint does not bill insurance. If submitting for reimbursement please confirm your policy benefits cover "Out Of Network" practitioners PRIOR TO YOUR SESSION. If covered, please have all information to be included on your receipt for reimbursement ready for your appointment. This will expedite the process significantly.
